Wanda, finding Tony to be heartless decides he should represent that so she slips into his room, traps him on his bed so he can't move or call for help. Then proceeds to use her powers to pull his heart out of his chest. Tony is awake during the process and is shocked as he feels the pain and sees it happening. Wanda then puts his still beating heart in a jar and leaves. Tony falls unconscious from shock.
The next day he awakens thinking it was a horrible nightmare, but when he lifts his shirt he sees the bloody hole where his heart once was, yet he's still alive! He races to the rest of the team to show them what Wanda has done, but they don't see anything odd which makes Tony think Wanda manipulated their minds not to see the hole and that he's literally heartless. Wanda pretends she doesn't know what he's talking about, and his A.I Jarvis detects nothing amiss. Tony knows it's impossible for him to be walking around without a heart so he thinks Wanda may have just manipulated his mind to see such a thing, yet why can he literally stick his hand in his chest and feel the emptiness? Why can he wander into Wanda's room and see his beating heart in a jar?
Whether Wanda literally stole his heart or just manipulated Tony's mind to think so is up to you :)
