John Garrett's system is pumping full of the new serum that has him seeing the universe.
The two little scientists on board are Coulson's pets and just tried to kill him. They're cute little things, who can't defend themselves against a normal person, let alone a super powered one.

He takes revenge on Coulson for all he's done, and on Fitz for his attempted murder, by torturing and raping Simmons. Possibly in their labs.

Bonus:

+ if Fitz is treated just the same as Simmons

++ He knows they're being recorded on security camera, knows it'll track sound and most likely they'll be watched later by SHIELD if it remains, so he makes one or both of them scream Coulson's name to hurt him.

+++ Ward doesn't try to help them, possibly taking a turn himself, but Raina is disturbed.

++++ Fitz forgets because of brain damage afterwards, and Jemma doesn't mention it until someone finds the security footage weeks later when clearing up old things.
